Required Medical Records

Anxiety Bipolar Disorder ADHD Depression PTSD Required Medical Records Mental health specialist report (e.g. psychologist or psychiatrist, report cannot be older than 5 years) – must state or confirm official diagnosis. Medication history – demonstrating that you have tried conventional medication for a period of 6 consecutive weeks (e.g. SSRI therapy for depression) – obtainable from your pharmacy. Optional additional records: Therapies and treatments to date; medical records from family doctor.
Required Medical Records Specialist report (e.g. neurologist or geriatrician) – must state or confirm official diagnosis. Medication history – obtainable from your pharmacy.
Knee, hip, or shoulder replacement surgery. Advanced arthritis of neck, spine and other joints. Required Medical Records Surgical report OR specialist report OR imaging report* – either one must state or confirm official diagnosis. Medication history – obtainable from your pharmacy.
Rheumatoid Arthritis Lupus Connective Tissue Disease Sarcoidosis Required Medical Records Specialist report (e.g. rheumatologist) – must state or confirm official diagnosis. Medication history – obtainable at your pharmacy.
Required Medical Records Specialist report (e.g. oncologist or pathologist) OR Imaging report*. Medication history – obtainable at your pharmacy.
Conditions can vary: including muscles, bone, joints, lumbar spine, or MSK injuries (shoulder, knee, or hip).
